Insurance Information
Doctors of Dental Medicine participates with Horizon Blue Cross/Blue Shield the traditional plan, and Delta Dental the premier plan.

Treatment recommendations are based on your dental needs not on your insurance coverage. We will do our best to give you an accurate estimate of the benefit you can expect to receive, however, it is just an estimate and ultimately it is your responsibility to know the limitations of your dental plan. You are expected to pay the difference between the estimated and actual insurance payment.

For our patients with dental insurance, please understand that insurance coverage varies greatly from policy to policy, as do co-payments and deductible. Some policies pay a fixed amount for services and others pay a percentage of the charge. Our fees are fair and competitive. As a courtesy to our patients we will accept assignment from your primary insurance carrier. You are responsible for any co-payments and deductible at the time services are rendered. Should your actual insurance payment differ from the estimated benefits, you will be billed or issued a refund accordingly. As a convenience we accept most major credit cards, personal checks, and of course cash. Other forms of outside financing are available upon request.

Filing Secondary Insurance
You may be covered by more than one insurance plan. If you have secondary insurance, we will not be able to submit that claim for you, unless your secondary carrier is Delta Dental or Horizon. If your secondary carrier is an insurance company other then Delta Dental or Horizon, you must submit your secondary claim on your own. After you settle your account with us we will be happy to assist you with submitting your secondary claim. Once they receive the claim your insurance company will reimburse you directly.
